In the early weeks of January 2012, a report of four cases of tuberculosis from Mumbai, India, stirred up a storm.1 India bears a giant's share of the world's multidrug-resistant tuberculosis (MDR-TB) burden, but these cases were different even though they came from a centre (Hinduja Hospital and Research Center) which has been reporting on the alarming escalation in drug-resistant TB in Mumbai over the last two decades. Pakistan’s enormous macroeconomic, internal, and human security challenges coexist alongside the opportunity created by a huge desire for change. With democracy taking root and a new constitutionally ushered era in state governance, The Lancet Series about Pakistan and health focuses on health as a nation-building and social-welfare agenda at a time of unprecedented social upheaval and economic hardships in the country. We call for a unified vision for the goal of universal and equitable health access. We provide recommendations for six objectives for policy and action.

Non-communicable diseases, including cardiovascular diseases, cancers, respiratory diseases, diabetes, and mental disorders, and injuries have become the major causes of morbidity and mortality in Pakistan. Tobacco use and hypertension are the leading attributable risk factors for deaths due to cardiovascular diseases, cancers, and respiratory diseases.

Pakistan has undergone massive changes in its federal structure under the 18th Constitutional Amendment. To gain insights that will inform reform plans, we assessed several aspects of health-systems performance in Pakistan. Some improvements were noted in health-systems performance during the past 65 years but key health indicators lag behind those in peer countries. 78·08% of the population pay out of pocket at the point of health care. The private sector provides three-quarters of the health services, and physicians outnumber nurses and midwives by a ratio of about 2:1.

Meeting slated for May 1, makes strong case for hydro to be declared as renewable energy, Jyotiraditya Scindia

Union Power Minister Jyotiraditya Scindia will chair an advisory group on May 1 to discuss the bottlenecks in the development of hydro power projects, falling share of hydro power, delays in environment and forest clearances and high capital costs.
The group, which also includes high-profile private sector players, is expected to make a case for hydro power to be declared renewable energy, by making it obligatory to increase its share to 20 per cent of total generation by the 15th Plan from the present level of 16 per cent.

Formation of Kerala Drinking Water Supply Company Ltd.

On April 9, Ponnath Devarajan, city councillor for Marad, a communally sensitive fishing hamlet that faces water shortage, moved a resolution before the Kozhikode Corporation Council against a government order for forming Kerala Drinking Water Supply Company Ltd.
He said the company, described by the Water Resources Department in its December 31, 2012, order as one modelled after Cochin International Airport Ltd., was a sheer effort to commodify water, a natural resource and public asset.

The Science, Technology and Innovation Policy 2013 sets a target to increase R&D expenditure to 2% of gross domestic product, this through public-private partnerships. But, compared to China, where is the commitment to R&D? India's innovation potential is grossly underutilised. The universities as centres of advanced learning and research suffer from official neglect. There has been little effort to set up technology parks linked to them in their vicinity.
